Schedule / Job Type

Contract | Days | 5x8-hour shifts (8:00–16:30) | Expected 40 hours/week

Key Responsibilities

• Provide occupational therapy services to an assigned caseload of 8–12 adult and geriatric home health patients
• Conduct evaluations, develop treatment plans, and deliver skilled OT interventions within the home setting
• Document patient care and progress using Cerner electronic health records
• Travel within a 35-mile service territory to complete patient home visits; mileage reimbursed at the federal IRS standard mileage rate
• Float within scope of practice to meet organizational needs as required
• Complete MOAB certification (or 4-hour de-escalation course during orientation week if not already certified); failure to pass may result in contract termination

Required Qualifications

• Active New Mexico Occupational Therapist license (pending licensure may be considered at submission)
• NBCOT certification required
• MOAB certification required (or must be obtained during orientation week)
• Current BLS certification required
• Must live at least 100 miles from the facility to be eligible; non-local driver's license required on file
• Second set of fingerprints required upon arrival for CYFD compliance (applicable to home health clinicians)

Preferred Experience / Skills

• Minimum 1–2 years of occupational therapy experience preferred; first-time travelers accepted
• Experience working with adult and geriatric patient populations in a home health setting
• Familiarity with Cerner electronic health records
